xml-attributes: documentation and change

While documenting xml-attributes:
http://www.w3.org/2003/g/xml-attributes

I realised that its ability to produce nested xml:base's was pretty 
limited (approx. zero), and that this is needed in some likely cases.

I have added "Advanced Usage" section to the documentation for such 
cases, which involves new functionality that I haven't yet implemented.
