Interestingly, in elaborating the default trigger issue, I realized that the way we were thinking a form author might "sometimes" make a default trigger is actually quite flawed as it actually creates an infinite loop! See http://www.w3.org/MarkUp/Forms/wiki/Default_trigger for details, but clearly this is where we will start since it is where we left off and since the infinite loop problem seems to make the issue more urgent.
