Coming phone calls: please fill in questionnaire

As usual at this time of year, the vacation period approaches, so also as  
usual, I have created a questionnaire which I ask you all to fill in, to  
say which calls you are planning on NOT attending. In that way we can see  
if any call is particularly at risk.
