Minor tweaks to DUV doc

I've been looking at the DUV doc today and seeing where I might be able 
to help. First thing I noticed was that ReSpec wasn't doing its job 
properly and was reporting some odd errors. I wanted to blame my 
colleague Eric Prud'hommeaux for that but that wouldn't be entirely 
fair. I think his script for the T/J switch was included twice, once 
externally and once within the doc. That upset things so that reSpec 
couldn't work but - it's OK now, so that references work (e.g. 
[[VOCAB-DCAT]].

Remember that it's easy to include references. Most of what you'll ever 
want to include a ref to is included by ReSpec. You can look up what is 
in its library at http://www.specref.org/

There was some text written into the HTML that I guess came from 
previous versions where ReSpec generates it (the doc status boiler plate 
stuff, the references etc.). I think that's all fixed now too.

The doc has many open issues and notes - which is fine - but are they 
all still relevant?

I'll generate the diff from the previous version before publication but 
if you could please add a little bit of editorial text to Appendix B 
(Change history) that would be good - i.e. give people a quick view of 
what has changed since http://www.w3.org/TR/2015/WD-vocab-duv-20150625/
