For the vocabs it would make sense to associate the vocab list. I believe the issue will be lower-level discussions most of the time, perhaps often editorial, so not a bad fit for what I understood you wanted to have the list for.
  
I have to say I'm a bit reluctant for creating issues for every item in a mail sent after a review, especially the editorial ones that require little discussion. This will be really a lot. We could have one issue per review mail, and then one derived issue when the mail really raises an issue that deserves individual discussion.
