Minor changes following our discussion yesterday

I have made some minor changes on the draft PWP and merged the branches. Ie, the editors' draft:

represents the latest version at this moment. Based on the discussion yesterday, the latest commit:

• Editorial changes in the section on identifiers (e.g., turned the definition list into separate section; better for addressability).
• Per Bill's remark the text makes it explicit that several identifiers may be included in the publication
• The subsection on internal resources includes only the 'scope' approach as discussed on the call, and has only a note for an alternative.
• Linking in the issue raised by Tim Cole[1] (as an open issue for now)

Please, give your comments and issues (or even your own editings and pull requests). I think it would be good to publish this sometimes next week, as a 'post TPAC' version.
