Peter, to start with, I have created a new questionnaire https://www.w3.org/2002/09/wbs/64149/DPUB-STEM-2014-12/ it belongs to the DPUB IG but it is public (it allows to answer the questionnaire without a W3C account). You can now build on this questionnaire.
