editorial changes to CSV2* completed

Finally (!) I have completed the editorial changes raised by 'timeless' and
captured in GitHub ISSUE #679.

I have asked Ivan to proof-read the changes & have asked timeless to
comment on their satisfaction.

Assuming all is OK, I think we can close this action.

Apologies for taking so long ... this task has been waiting for it's slot
to get to the top of the pile!
