open issues for csv2* docs to be resolved

Hi - we didn't discuss this issue at the teleconf today (ran out of time).

I note that "Include @context in JSON output?" #615 [1] is still open.
Looks like there's consensus except for @gkellogg.

Given the need to finalise the documents for transition to LCCR, can we
close this or do we need to make a change?
