New version of MathCAT with "say cap" and pitch change

I have uploaded a new version of MathCAT to the demo site
<https://nsoiffer.github.io/MathCATDemo/>. In addition to bug fixes to
recognize more mhchem (\ce{...}) output, I also added three features we
discussed on the call:
1. There is now a checkbox "say cap" that controls whether pitch change is
used or "cap" is used.
2. I changed the demo to act like screen readers that automatically change
the pitch of capital letters.
3. I made the speech different for terse, medium, and verbose settings.

Please let me know what you think. In particular, is "say cap" really
needed?

If you find bugs or speech that you think should be improved, please let me
know ASAP. I want to release a new version of the MathCAT NVDA addon soon
that incorporates chemistry.
